Sustainable patient outcomes require sustainable practitioners.

Most practitioners aren’t lacking knowledge or skill.
They’re navigating constant demand, emotional load, and pressure to deliver—without the support to sustain it.

When the practitioner is depleted, even the best plan doesn’t get implemented.

Before we talk strategy, let’s identify what’s getting in your way.

Is burnout quietly building in your practice?

  • You’re working late, catching up on notes, telling yourself:
“I’ll make time for self-care next week.”
    But next week never comes.

  • You know you need time off, but can’t shake the fear your practice will fall apart without you.
    So you keep pushing.

  • Your schedule is full—you’re moving from one task to the next without a moment to breathe.
And it’s catching up with you.

  • You’re giving everything to everyone else…
but starting to feel invisible yourself.

Burnout is what happens when your good intentions run on empty.

It doesn’t happen to the careless.
It happens to the committed, the conscientious, the ones who care deeply.

You don’t need a new career—you need a new way to work.

  • Enjoying your work and still having time for the things you’ve been putting off

  • Waking up energized, not already behind

  • Setting boundaries without guilt—and actually honoring them

  • Leading your practice with clarity, not constant pressure

Let’s rewire your approach for resilience, clarity, and sustainable energy.

It’s about thinking differently so what you do actually works.

You give everything to your patients and clients—but what about you?

The problem isn’t a lack of knowledge.
It’s the gap between knowing what to do—and having the capacity to follow through.

This work integrates Positive Intelligence® (PQ) with practical implementation support to help you build mental fitness in real time.

Using simple, neuroscience-based techniques, you learn to:

  • Shift out of constant catch-up mode

  • Stay calm, focused, and clear under pressure

  • Make sustainable changes—without overhauling your entire life

So the care you provide to others…
finally includes you.

The Implementation System for Sustainable Change

This work goes beyond education.
It provides a structured, supportive way to turn insight into
real-life implementation.

You’re not just learning what to do—
you’re developing the mental fitness and practical systems needed to follow through.

Using Positive Intelligence® (PQ), lifestyle medicine principles, and guided implementation, you begin to:

  • Recognize and interrupt patterns that lead to stress and burnout

  • Strengthen your “Sage” response for clarity, focus, and better decision-making

  • Apply small, meaningful shifts that compound into lasting change

As part of this process, you receive personalized Sage At Work™ guides based on your unique saboteur patterns and strengths.

These tools help you:

  • Navigate real-world challenges in the moment

  • Lead, decide, and respond with greater intention

  • Integrate mental fitness into your daily work—not just your free time

Make what works—work better….

So change doesn’t live in theory—
it becomes part of how you think, work, and lead every day.

Hi, I’m Antoinette “Toni” St. Clair-Fish FMCHC, NBC-HWC

I spent years building a successful career—on paper, everything looked right. but behind the scenes, I was exhausted—physically, mentally, and emotionally.

The long hours, constant pressure, and never-ending demands were taking a toll I could no longer ignore.

When I reached my breaking point, I knew something had to change.

That moment set me on a path to reclaim my health, my energy, and a way of working that actually felt sustainable.

I immersed myself in functional medicine, lifestyle medicine, and mental fitness—not just to recover, but to understand how real change actually happens.

What I discovered:

Sustainable success isn’t about pushing harder—
it’s about aligning how you work with how you’re wired.

I built a thriving career—then realized I was running on empty.

Now, I help high-achieving professionals and healthcare practitioners do the same—

without burnout, overwhelm, or sacrificing what matters most.

From Burnout to Sustainable Health

When Dr. Toyin Alakija came to me, she was burned out, overwhelmed, and neglecting her own well-being. Work consumed her life, leaving little time for her health, passions, or family.

I helped her prioritize self-care and emotional resilience, setting boundaries, embracing mindfulness, and letting go of perfectionism.

Now, she feels clear, strong, and aligned with her values—balancing her career, health, and personal life with confidence and ease.

“Working with coach Toni has been such a transformative experience! She created a safe space for me to explore my challenges without judgment and provided accountability to see projects through. That process has led me to make more progress in my personal and professional life than I ever expected! - Dr. Toyin Alakija, Physician

What Mental Fitness Looks Like in Real Life

What You Might Be Wondering

  • No worries! Life happens. While live participation is encouraged for the best experience, you’ll still have access to the PQ app, video modules, and ongoing support in the private group chat to stay on track. Plus, you can check in with your group between sessions to catch up on key takeaways.

  • This program is focused on mental fitness training—helping you rewire your brain for resilience, confidence, and clarity using Positive Intelligence® (PQ®). Unlike therapy, which explores deep-rooted emotional issues, or traditional coaching, which often focuses on external goals, this program trains you to shift how you think and react in real-time, building lasting mental habits for stress management, emotional regulation, and personal growth.

  • You’ll have full access to the PQ app, guided practices, and video modules throughout the entire 12-week program. After that, you’ll still be able to apply the tools you’ve learned for 9 months. That’s a full 12 months!

  • This program is designed to fit into a busy schedule—no overwhelming time commitments here! Expect to dedicate:
15 minutes per day for guided mental fitness exercises (PQ reps)
1 hour per week for self-paced video modules (during the intensive phase)
30-45 minutes per group coaching session (weekly for the first 6 weeks, then bi-weekly)
That’s about 2-3 hours per week—a small investment for a major shift in your resilience, clarity, and well-being!

  • Both! While this program is perfect for individuals looking to strengthen their mental resilience, teams and organizations can also participate. This is especially valuable for healthcare teams, wellness practitioners, and leadership groups looking to improve stress management, communication, and overall workplace well-being. If you're interested in a team experience, let’s chat about a customized approach!

  • That’s exactly where this work begins.

Most of the people I work with are knowledgeable, committed, and already doing “the right things”, but are not doing things right. The challenge isn’t information—it’s having the capacity, support, and systems to follow through consistently in real life.

This work helps you bridge that gap—by strengthening your mental fitness, identifying what’s getting in your way, and creating practical ways to implement what you already know.

So instead of starting over, we make what already works—work better.Item description
